Insurance
We accept CareOregon/Oregon Health Plan (OHP). We see people with or without insurance. If you do not have insurance and are interested in OHP coverage, schedule an appointment and we can help you apply. Contact us at 503-645-9010 to get started.
We accept Trillium Community Health Plan and some commercial insurance for limited programs.
Sliding scale
We believe cost should never stop you from getting care. If you don’t have coverage or need lower costs, we offer a sliding fee scale. Your cost is based on income and family size. We want to make care affordable for you.

Sliding fee scale rates
If you do not have insurance, you may qualify to pay lower rates based on a sliding fee scale. How much you pay depends on your family size and income.
Step 1: Find your tier
- First, find the number of people in your household under the Family Size column. Count the people you would include on your taxes.
- Example: If you live with a spouse and two children, your family size is 4 people.
- Next, find your monthly income in that same row. The column
Your income falls under is your tier.- Example: If your monthly household income is $3,000 and your family size is 4, you are in Tier B.

If you make more than these amounts, you will pay the full fee.
