LifeWorks NW Milwaukie Outpatient site team recently moved to a new location on the Providence Hospital Milwaukie Campus. The new address is 10202 SE 32nd Ave., Suite 701, Milwaukie, OR 97222, as of September 26, 2023.
Our International Way building offered clients and staff a comfortable environment but was not easily accessible for those using public transportation. The Providence Milwaukie Hospital campus is easy to reach with frequent buses and is approximately a mile from our old site.
By serving clients next to the hospital, clients will have greater opportunities for multiple appointments.
The hours remain 8 am to 5 pm, Monday through Friday. The previous front desk phone number was transferred to the new site: 503-496-3201. If you have questions, please call us there.
